Comprehensive Ear, Nose, and Throat P.C. will not turn a patient away because of his/her insurance. We do require, however, that payment be received at the time of service if we do not participate with your insurance. We will then provide you with the information that you will need to submit to your insurance company for reimbursement. We accept cash, check, Mastercard or Visa.
We ask that you bring your insurance card(s), co-payment and a list of prescriptions and drug allergies to your scheduled appointment time. If your appointment was made more than a week before your scheduled date, you will receive a paperwork packet in the mail. Please complete this before your scheduled appointment time.
We ask that you arrive 15 minutes prior to your scheduled appointment time. This amount of time is necessary for us to process your insurance information and ready your medical records for the physician. Please enjoy our complimentary coffee and selection of current magazines while you wait.
In order to maximize the effectiveness of your visit here, it is important that our physicians can view any and all radiology films and reports that you have had. You will need to call the hospital film room where you had your scan performed and request that a CD containing your films be mailed to our office. If there are time constraints, you can also pick up the CD at your convenience. Arrangements must be made through the hospital when requesting a CD.
If you have Workman's Compensation or Motor Vehicle Accident insurance, we ask that you bring the name of your primary contact at the insurance company, their phone and fax numbers, billing address, your claim number, the date of your injury and any other pertinent information that you have available. If we do not have the above information, payment will be due at the time of service. We will then provide you with all the information you will need to submit your claim.
If you have Medicaid or Priority Health Medicaid your primary care physician must call to make your initial appointment and any follow up appointments you may need. By having your primary care physician call our office, this ensures that they are aware of and have approved the appointment. If you have Children's Special Health Care Services (CSHCS), we will need to have a referral on file. If our physicians have been added to your CSHCS plan, we will receive a referral valid for one year.
All children under the age of 18 must attend their appointment with a parent or guardian present. If a parent or guardian cannot attend the appointment, a grandparent or family friend who is over the age of 18 may bring them, with authorization from their parent or guardian. This authorization must be in written form, signed by the parent or guardian, and presented at the time of service.

You no longer need a referral from your insurance company to see our physicians. It would be wise, however, to notify your primary care physician of your appointment here. Records may be mailed or faxed.
We request 24 hours notice when canceling or rescheduling your appointment. This allows for us to fill your appointment time with individuals who need immediate medical care. Please be considerate and give us as much notice as possible when rescheduling or canceling your appointment time.
Our staff will gladly preauthorize your surgical procedure with your insurance company. This will be done approximately one week prior to your surgery date. Our billing department will contact you if we foresee an out of pocket expense for you and your family. Your surgery will be scheduled once the authorization is received.
If you are having surgery at Metro Health, Spectrum, St. Mary’s, or one of the Surgical Centers, they will contact you one to two days prior to your scheduled surgery time. If you have a concern, and need to speak to the hospital, call the pre-procedure planning department by calling the number listed on the back of your brochure that was given to you when you scheduled your procedure date.
We ask that you bring a friend or family member who can communicate on your behalf to your appointment. If there is no one available to assist you, please call our office to make arrangements.

We would be happy to set up a payment plan to assist you in paying your bill. Payments can be made monthly and must be arranged with the billing specialist.
You may request your medical records by mailing or faxing a written request or release to our office. Please indicate where you would like the records sent, the address or fax number, what reports or documents you would like and the date you need them. There will be a $12.00 charge for records four years or older that need to be pulled from storage. Attorneys, insurance companies and the State of Michigan will be charge for medical records. There is no charge for patient and physician requests.
