Before your first visit to the office, we request that you kindly fill the two forms for your demographic information as well as your review of health conditions found below. Please bring these with you for your first visit.
PRE-OPERATIVE INSTRUCTIONS:
There are specific guidelines to follow before your planned surgery. Firstly, our office staff will instruct you about the date chosen. If the appointment was made while you were in the office, a set of instructions was already given to you. Alternatively, these will be mailed to you. Additionally, you can find the forms below.
**Kindly follow the instructions carefully. Please also be careful to obtain all necessary laboratory testings and medical clearance prior to your surgery.
POST-OPERATIVE INSTRUCTIONS:
Postoperative instructions are provided for some of the more common procedures performed by our physicians. Kindly download and print those concerning your procedure. If you still have any questions, please do not hesitate to contact our office for additional guidance. .

Comprehensive Ear, Nose, and Throat P.C. accepts all major types of insurance. For specific questions, please contact our office directly. We will be more than happy to provide you with our most current insurance participation.
The bills you receive as a result of the care provided by Comprehensive Ear, Nose, and Throat P.C. may be confusing. The following information will be helpful for you as you read through your bill. For insurance policy questions about specific covered charges, patients should contact their insurance carrier.
Primary and secondary insurance carriers will be billed. It is important for patients to keep our office informed about all their health insurance plans. Insurance information provided by patients will be verified. It is important that Medicare patients list their supplemental insurance policies as well.
Co-pays and deposits may be required before surgery. We will notify patients of any deposit requirement as soon as our office has verified their insurance coverage. For convenience, we accept cash, checks, and money orders, as well as Visa, Mastercard, and Discover credit cards.

Participation with an insurance plan means that Comprehensive Ear, Nose, and Throat P.C. will bill and collect for covered services. Patients are responsible for any prior authorization or approval, if required by their insurance policy or managed care program. Patients are also responsible for checking with their insurance company and/or employer to obtain and complete any required forms. Any deductible and co-insurance amounts, as well as any other non-covered charges, are the patient's responsibility. Our office participates will all major insurance plans, however, patients should contact their insurance carrier for information regarding current participation guidelines.
